Thomas Heijligen 0f51f62c62 hwaccess_x86_io: clean header concept
Move all function implementations into the .c file

TEST: `[g]make [WARNERROR=no]` on Linux, FreeBSD, NetBSD, OpenBSD,
DragonflyBSD, OpenIndiana, Debian-GNU/Hurd
