Endian conversion: move to platform.h and platform/endian*.c
Starting to move the platform dependent code to platform/ and provide
the abstraction through the platform.h header.

hwaccess_physmap: add missing DirectHW include
For MACH / APPLE map_physical is defined in DirectHW.h
TEST: run `make`
compiles,
linking fails due to missing directhw object in my setup, don't know
how to fix this

hwaccess: move mmio functions into hwaccess_physmap
The mmio_le/be_read/writex functions are used for raw memory access.
Bundle them with the physmap functions.
